Goal: The goal of this program is to educate during every visit, to assist patients in developing a longitudinal personal record of medical history and care plans, and to provide a tangible way to engage patients in their own care. The overarching goal was better compliance, recognition of medication side effects, and improved adherence to specific and agreed upon lifestyle changes.

Impact: Sixty percent of patients participate in care plan tracking with a health notebook, and 80% percent of patients complete a prep form to help organize visits.

Goal: To describe a new model of enriched supportive housing that not only improves the quality of life of seniors but also can provide a return on investment that reduces health care expenditures.

Impact: Housing chronically homeless adults reduces homelessness, improves health outcomes, and reduces health care costs. The greatest reduction in health care costs after placement in supportive housing is seen among chronically homeless adults and seniors.

Goal: The goal of the MEND program is to reduce obesity levels in children by offering free healthy living programs that aim to encourage small lifestyle changes that improve health.

Impact: The MEND program was successful in reducing waist circumferences and BMI scores while increasing cardiovascular fitness, physical activity, and self esteem in children placed within the intervention group. The results of this study suggest that the MEND program is a promising intervention to combat rising child obesity rates.

Goal: The program has three primary goals:
1) to improve pregnancy outcomes by promoting health-related behaviors;
2) to improve child health, development and safety by promoting competent care-giving; and
3) to enhance parent life-course development by promoting pregnancy planning, educational achievement, and employment.

The program also has two secondary goals: to enhance families’ material support by providing links with needed health and social services, and to promote supportive relationships among family and friends.

Impact: Evaluations of the program have shown that women who were visited by nurses had significantly better outcomes than those who did not in terms of measures such as maternal health, maternal life-course development, child health and safety, and adolescent measures of delinquency.

Goal: To prepare children of disadvantaged families for academic success and to strengthen families through intensive home visiting.

Impact: The Parent-Child Home Program builds school readiness, starting from the home. PCHP utilizes a non-directive, in-home modeling approach that encourages children's development, builds meaningful relationships between parents and children, and allows underserved families to access educational early-childhood services.

Goal: The Goal of this Promising Practice is to determine whether a tailored community health worker (CHW) intervention would improve post-hospital outcomes among low-SES patients.

Impact: This intervention would improve access to primary care and quality of discharge while controlling recurrent readmissions in a high-risk population. Health systems may leverage the CHW workforce to improve post-hospital outcomes by addressing behavioral and socioeconomic drivers of disease.
